    Complete before your first visit

    To make your first visit as efficient as possible, please complete the following forms before your appointment. You may print and complete them at home, or arrive 15 minutes early to complete them in our office.

    Patient Registration Form

    Required

    Basic contact information, emergency contacts, and demographic details.

    Medical History Questionnaire

    Required

    Comprehensive health history including conditions, surgeries, and hospitalizations.

    Current Medications List

    Required

    All prescription medications, over-the-counter drugs, vitamins, and supplements.

    HIPAA Privacy Authorization

    Required

    Acknowledgment of our privacy practices and consent for treatment.

    Financial Policy Agreement

    Required

    Understanding of payment policies and financial responsibility.

    Consent for Treatment

    Required

    Authorization for examination, diagnosis, and recommended treatment.

    Pre-Surgery Guidelines

    If Receiving Sedation or General Anesthesia

    Critical instructions for your safety

    Do not eat or drink anything (including water) for 8 hours before surgery
    Take prescribed medications with a small sip of water only
    Wear comfortable, loose-fitting clothing with short sleeves
    Remove contact lenses, jewelry, and nail polish
    Do not wear makeup on the day of surgery
    Arrange for a responsible adult to drive you home
    Someone must stay with you for 24 hours after surgery
    Medications
    Continue taking prescribed medications unless specifically instructed otherwise
    Inform us of ALL medications, vitamins, and supplements you take
    Blood thinners (Aspirin, Warfarin, Plavix) may need to be adjusted prior to surgery
    Bring a complete list of your medications to every appointment
    Ask about any medications you're unsure about
    Day of Surgery
    No smoking for at least 12 hours before surgery
    Brush your teeth the morning of surgery
    Arrive 15 minutes before your scheduled appointment
    Bring your insurance cards and photo ID
    Wear closed-toe shoes for safety
    Leave valuables at home

    Transportation Requirements

    If you are receiving IV sedation or general anesthesia, you must have a responsible adult (18 years or older) accompany you to your appointment. This person must:

    • Drive you home after the procedure
    • Stay with you for 24 hours following surgery
    • Be available to receive post-operative instructions
    • Help administer medications if needed

    Uber, Lyft, taxi, or public transportation are not acceptable alternatives. Your procedure may be cancelled if you do not have an appropriate escort.

    Post-Operative Care Overview

    Rest & Recovery

    Plan to rest for the remainder of the day. Avoid strenuous activity for 3-5 days depending on your procedure.

    Medication Management

    Take prescribed pain medication and antibiotics as directed. Use ice packs to reduce swelling.

    Diet Modifications

    Start with clear liquids and soft foods. Avoid hot foods/drinks, straws, and smoking for at least 24 hours.

    Follow-Up Care

    Keep all follow-up appointments. Contact us immediately if you experience excessive bleeding, fever, or severe pain.
